45歳、ポルトガル女性

右手第5指の痛みを訴えて救急科へ

第5指と第1指、母指球にあざ様の浮腫と変色有り

定期的に服用する薬剤なく

同様のエピソードもなし

身体所見；

爪の毛細血管充満試験は正常

左右上肢の血圧に差はなし

アレンテスト陰性

変色は、寒冷暴露との関連なし

血液検査でも、特に異常なし

患者は、5日後に自然寛解

臨床経過などから、アッヘンバッハ症候群と診断された。

アッヘンバッハ症候群は、原因不明の発作性指血腫で、あざを伴う突然の痛みが起こり数日以内に消退することが多い。

特別な予防や治療法はなし

痛みとあざを訴えることが多い

半数が腫れも

1/3に知覚異常

左右差はやや右手に多い傾向がある

第2指＞第3指＞第1指の順で多い
