SLR試験について
SLR(Straight-leg-raise)試験の診断特性についてです。
ラセーグ徴候と同義で扱われることが多いようですが、
これらは、下肢への放散痛を検出するために用いられる身体所見です。
われわれ鍼灸師や柔道整復師の先生がたにとっても有名な身体所見の1つですが、
その意味や診断特性についてご存じでしょうか?
本日は、その報告をいくつか集めました。

まず、SLR試験とラセーグ徴候は厳密には同義ではありません。
その意味を知るには、少し遡った歴史を知る必要があります。
SLR試験は、Lazarevic博士によって、初めて公表(1880年)されました。翌年(1881年)にラセーグ博士の弟子であるForestがラセーグ徴候を記載し、1969年にWilkinsが英訳したものが、徐々に誤った解釈で広まってしまったとされています。
どう間違えたのか?
SLR試験は、Lazarevicによると坐骨神経痛の誘発テストとして、坐骨神経の伸展により起こる再現痛としています。
一方、ラセーグ徴候も坐骨神経痛の誘発テストですが、2つの手技を行い、坐骨切痕高位での疼痛誘発で陽性なので、臀部の筋の収縮による坐骨神経の圧迫とされています。
この手技の1つがSLR試験と同じ方法のため、間違えやすいのですが、
SLR試験は、膝より末梢の放散痛を指すが、ラセーグ徴候は、臀部の痛みを指す。
疼痛がどこに誘発されるか?を診てる場所が違います。
原著での報告とはいつの間にか変わり、混同され、報告によりそれぞれのテストの定義があやふやになりました。
腰部痛で陽性とするもの、腰痛+臀部痛で陽性、腰痛+下肢痛など、様々です。
また足の挙上角度で陽性とするかも変わるとされています。
日本整形外科学会では70度以下ですが、80度や60度などもあります。
このように、どこの痛みか?挙上角度は?などで判定は当然変わってきます。
そのため、診断精度も報告により違いがでてきます。

どういう基準で陽性にしているかは別として、報告のほとんどで特異度ならびに陽性尤度比(LR+)は高くありません。
どちらかといえば、感度や陰性尤度比(LR-)の方が良好なようです。

これらの報告で疼痛誘発が腰部なのか?臀部も含む?下肢痛?下肢痛ならばどこまで?
といった厳密な記載があるものもあれば、ないものもあります。
なので、SLR試験を行う先生は、自分の考えるもの(下肢のどこの放散で陽性とするか?など)と近い診断精度を探して、考えておく必要があります。
全体的に、SLR試験は、腰椎椎間板ヘルニアの確定所見ではなく、除外試験としての方が有用そうです。
